2024-25 SPORTS PHYSICAL INFORMATION
The directions for signing up for sports physicals for the 2024-2025 school year and a copy of the PIAA physical form that must be completed to get on the list are attached.
The Monessen Athletic Department will have Sports Physicals for the 2024-2025 school year on Friday, July 26, 2024, from 8:00 AM to 2:00 PM in the Nurse's Office. In order to be placed on the list, you must return a completed PIAA Physical Form to Ms. Naccarato by Wednesday, July 24, 2024, by 2:00 PM.
